Patient info

First Visit

Your initial appointment will consist of a one or two appointment examination and consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. A complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. Please assist us by providing the following at the time of your consultation:

1. New patient registration forms
2. Any referral slip or x-rays
3. List of medications you are presently taking
4. Medical or dental insurance

A parent or guardian must accompany all patients under 18 years old at the consultation visit. Please alert the office if you have a medical condition that may be of concern (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.), or if you are on any medication (i.e. heart medication, aspirin, anticoagulant, etc.), or require medication before dental cleanings (i.e. antibiotics for pre-med).

If your previous dentist has taken recent x-rays within six months to one year, you may request that they are available to you or forwarded to our office. If additional films are necessary, they can be taken at our facility. A full mouth, panoramic x-ray or a dental CT scan is essential to complete a comprehensive exam.

Insurance and Financial Policy

Our office accepts all dental plans that allow patients to see dentists of their choice (PPO, DPO) and helps patients maximize their benefits. We will process dental claims on your behalf, track the insurance payment progress, and provide you with an estimate for each dental procedure through insurance pre-authorization. Please bring your insurance information to the initial consultation.

For your convenience, we also accept Amex, Visa, Mastercard and Discover. Payment is due at the time service is rendered, unless other arrangements have been made in advance. While we do accept insurance, patients are entirely responsible for all fees regardless of insurance coverage. Monthly payment plans are also available but must be discussed before procedures.

We also offer Care Credit as an additional financing option. Please click here to start an application. You can also make a payment by clicking here.
